CARE PLANS/CHRONIC DISEASE

For any patients who experience chronic illness such as heart disease, arthritis, kidney disease or any other condition that lasts for more than six months, we offer a supportive nurse-led clinic to ensure your care is optimal.

 

In conjunction with your GP, you will receive a written care plan outlining your health goals, activities and other medical or allied health specialists who are involved in your care.

 

Chronic Disease appointments are bulk billed with no out of pocket cost for all patients.

 

 

 

 

 

 

What is my Care Plan folder? 

When you have a Care Plan we will provide you with a coloured folder where you keep all your related information and EPC vouchers, 

How many vouchers do I get per calendar year?

For each calendar year you will receive 5 vouchers to use. If you do not use all 5 vouchers they will not carry over to the following year. 

Can I ask my doctor for scripts/referrals in my cdm appointment? 
 

If you require any other medical attention aside from your CDM related consult please let reception know when booking your appointment so they can allot an appropriate time frame. 

Life Program 

Are you at risk of developing type 2 diabetes, heart disease or stroke, have a BMI >35?

Then this is the program for you! 

 

The life program is a FREE program that is ran fortnightly for 8 weeks. During this time we discuss lifestyle changes that can dramatically reduce your risks of developing type 2 diabetes, heart disease and stroke. 

 

The program includes a session with a dietician to discuss food labelling and better food choices and a session with a physiotherapist to explain ways we can get moving at home. 

 

 

 

Health Assesments

Health Assessments are provided at our clinic year-round, with the most common assessments being for over 75s and patients aged between 45-49 years of age. 

 

These assessments provide a structured way of identifying health issues and conditions that are potentially preventable or amenable to interventions in order to improve health and/or quality of life.

 

How long will my CDM appointment take? 

CDM appointments usually take between 30 minutes to an hour. This incorporates a consult with the chronic disease nurse and your doctor.
